Creatine
This is the one that surprises people. Creatine isn't just for gym bros. It's quietly become one of the best-studied supplements for women in the menopause transition. It's also one of the best perimenopause supplements you've probably never been told about.
The evidence. A 2026 meta-analysis in the Journal of the International Society of Sports Nutrition pooled seven randomized trials (608 women). Creatine monohydrate increased lean mass, especially alongside resistance training (JISSN).
A 2025 trial called CONCRET-MENOPA gave creatine to 36 perimenopausal and menopausal women for eight weeks. The medium-dose group improved reaction time and raised creatine levels in the frontal brain (Journal of the American Nutrition Association).

Over-the-counter DHEA and pregnenolone. These are hormone precursors, not vitamins. They belong under practitioner supervision with lab monitoring.

- Omega-3: 8–12 weeks; tissue levels change slowly.
If you've given something 12 weeks at a sensible dose and nothing has moved, it isn't your supplement. Stop it and move on.
How to Build Your Perimenopause Supplement Stack
Step 1: Get a baseline. Pick your three most disruptive symptoms and score them 1–10 every day for one week before you change anything. "I think I'm sleeping better?" isn't data. A number is.

How to Buy Perimenopause Supplements That Are Actually What They Say
Supplements aren't regulated like medicines. Two bottles of perimenopause supplements with the same name can contain very different things. Five things to check on the label:
- Third-party testing. Look for a USP, NSF, or Informed Choice mark. It means someone outside the company checked what's inside.
- Standardized extracts for botanicals. Black cohosh should be a standardized extract, not "root powder." Ashwagandha should be a root extract with the withanolide percentage listed.
- Elemental dose for minerals. "Magnesium glycinate 1,000 mg" might deliver only 100–140 mg of actual magnesium. The elemental number is the one that matters.
- Single ingredients over blends. You can't troubleshoot a 12-ingredient "perimenopause support" formula. Buy the one thing you're testing.
- No "proprietary blend" on the label. That phrase legally lets the maker hide the dose of each ingredient.
Price is a poor guide to quality. Testing marks and disclosed doses are good ones.
